1. OPERATOR AND INTERMEDIARY

The operator is the company Poliklinika Trenčianska s.r.o with its registered office at Trenčianska 53/B, 821 09 Bratislava – Ružinov city district, Company ID: 56 243 197, registered in the Commercial Register of the Municipal Court Bratislava III, Section Sro, File No.: 178425/B (hereinafter referred to as the “OVB” or as the “operator”). The controller has designated a responsible person pursuant to Art. 37 et seq. of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (hereinafter referred to as the “GDPR Regulation”) and pursuant to Act No. 18/2018 Coll. on the protection of personal data, as amended (hereinafter referred to as the “Privacy Act”), who can be contacted in writing at the address of the controller’s registered office, by telephone at +421 911 717 818 or by e-mail sent to the address recepcia@poliklinikatrencianska.sk

2. PURPOSE AND LEGAL BASIS FOR PROCESSING PERSONAL DATA

The legal basis for processing personal data is: a) Art. 6 para. 1 letter b) of the GDPR in conjunction with § 13 para. 1 letter b) of the Personal Data Protection Act, according to which the processing of personal data is necessary for the performance of a contract to which the data subject is a party or for taking steps prior to entering into a contract at the request of the data subject, b) Art. 6 (1) (f) of the GDPR in conjunction with Art. 13 (1) (f) of the Data Protection Act – legitimate interest of the controller – in the case of processing client contact data (e.g. client email address) for the purpose of direct marketing (e.g. sending a newsletter).

3. CATEGORIES OF RECIPIENTS AND TRANSFER OF PERSONAL DATA

The recipient of personal data provided upon conclusion of the Purchase Agreement and/or expression of interest in receiving the Operator’s newsletter by providing the client’s contact details (e.g. providing the client’s email address) may be a company with property and/or personnel connections with the Operator, the Slovak Trade Inspection, a legal entity ensuring delivery of the shipment, lawyers, notaries, criminal prosecution authorities, courts, bailiffs and other public authorities. The Operator does not intend to transfer the personal data of the data subject to a third country or international organization.

4. PERIOD OF PROCESSING PERSONAL DATA

Operator at: a) the purpose according to point 2 letter b) of this Information processes the personal data of the data subject for a period of 3 years from the provision of the client’s contact details in the scope of the email address and telephone number.

5. RIGHTS OF THE DATA SUBJECT

The data subject has the right to: a) request access to personal data concerning him/her – request whether personal data are being processed about him/her, for what purpose they are being processed, what personal data are being processed about him/her, information about the recipients, the retention period or the criteria for determining them, information about his/her rights as a data subject. For repeated provision of personal data requested by the data subject, the controller may charge a reasonable fee corresponding to the administrative costs, b) to correct their personal data or to complete incomplete personal data, c) to erase his/her personal data or to restrict the processing of his/her personal data. The controller is obliged to inform the data subject before the restriction of the processing of his/her personal data is lifted, d) object to the processing of his/her personal data carried out for the purpose of the legitimate interests of the controller, including profiling, e) object to the processing of his/her personal data for the purposes of direct marketing, including profiling to the extent that it is related to direct marketing. If the data subject objects to the processing of personal data for the purpose of direct marketing, the controller will no longer process the personal data of this data subject for the purpose of direct marketing, f) the portability of personal data to another recipient, if technically possible, g) withdraw your consent to the processing of personal data at any time, h) submit a proposal to the Personal Data Protection Office to initiate proceedings on personal data protection, the purpose of which is to determine whether the rights of natural persons have been violated in the processing of personal data (Section 100 of the Personal Data Protection Act). The operator does not carry out automated processing of personal data, including profiling, to which the data subject has the right to object.
